Appears 5 times in the Quran.

1. 3:127 لِيَقْطَعَ طَرَفًا مِّنَ ٱلَّذِينَ كَفَرُوٓا۟ أَوْ يَكْبِتَهُمْ فَيَنقَلِبُوا۟ خَآئِبِينَ

   That He may cut off a portion from among those who disbelieve, or abase them so that they should return disappointed of attaining what they desired.
   

2. 14:15 وَٱسْتَفْتَحُوا۟ وَخَابَ كُلُّ جَبَّارٍ عَنِيدٍ

   And they asked for judgment and every insolent opposer was disappointed:
   

3. 20:61 قَالَ لَهُم مُّوسَىٰ وَيْلَكُمْ لَا تَفْتَرُوا۟ عَلَى ٱللَّهِ كَذِبًا فَيُسْحِتَكُم بِعَذَابٍ وَقَدْ خَابَ مَنِ ٱفْتَرَىٰ

   Musa said to them: Woe to you! do not forge a lie against Allah, lest He destroy you by a punishment, and he who forges (a lie) indeed fails to attain (his desire).
   

4. 20:111 وَعَنَتِ ٱلْوُجُوهُ لِلْحَىِّ ٱلْقَيُّومِ وَقَدْ خَابَ مَنْ حَمَلَ ظُلْمًا

   And the faces shall be humbled before the Living, the Self-subsistent Allah, and he who bears iniquity is indeed a failure.
   

5. 91:10 وَقَدْ خَابَ مَن دَسَّىٰهَا

   And he will indeed fail who corrupts it.
